About the Test
Indirect bilirubin, also called unconjugated bilirubin, is the form of bilirubin the liver has not yet processed. It rises when red blood cells break down faster than usual or when the liver is slow to take bilirubin up. This makes the test useful for investigating jaundice, unexplained fatigue, or dark urine, and for telling apart causes such as haemolysis, mild inherited conditions like Gilbert's syndrome, and liver-related problems.
What is it? A blood test measuring unprocessed (unconjugated) bilirubin, also known as indirect bilirubin or IBIL.

Indirect Bilirubin test: purpose, preparation and results
Why take the Indirect Bilirubin test?
- Investigate yellowing of the skin or eyes (jaundice) and find its likely cause
- Check for increased red blood cell breakdown (haemolysis)
- Help identify mild inherited patterns such as Gilbert's syndrome as part of a liver assessment
How to prepare for the Indirect Bilirubin test
No fasting is usually required for indirect bilirubin unless your doctor has combined it with a fasting liver or metabolic panel. Avoid prolonged fasting right before the test, as going without food for a long time can itself raise bilirubin.
Understanding your Indirect Bilirubin results
A high indirect bilirubin often reflects faster red cell breakdown or reduced processing by the liver, and can appear in harmless inherited patterns as well as in conditions needing attention. Your doctor-reviewed report is typically ready in 1 day and should be interpreted with your total and direct bilirubin and your symptoms.

This is a fast turnaround test. Your doctor-reviewed report is usually ready within 1 day of collection and is sent to you on WhatsApp and email, so you can share it with your physician quickly.
Generally no, but avoid a long fast just before the draw, because prolonged fasting can raise indirect bilirubin on its own. If it is bundled with a fasting liver or lipid panel, follow the fasting instruction for that panel.
It usually means red blood cells are breaking down faster than normal, or the liver is taking up bilirubin slowly. This can occur in harmless inherited patterns like Gilbert's syndrome or in conditions your doctor will want to assess further.
Indirect (unconjugated) bilirubin has not yet been processed by the liver, while direct (conjugated) bilirubin has. Comparing the two helps your doctor tell whether jaundice is coming from red cell breakdown or from a liver or bile-duct issue.
Many people with a mildly raised indirect bilirubin, such as those with Gilbert's syndrome, have no symptoms at all and are perfectly well. The finding is often picked up incidentally, and a doctor can confirm whether any follow-up is needed.
Yes. Our DHA-licensed nurses draw samples at home, office, or hotel across Dubai, with same-morning or next-day slots. The visit takes around 20 minutes and payment is self-pay in AED.
