What sciatica is — and what physio does about it

Sciatica is pain that runs from the lower back or buttock down the back of the leg, often with tingling or numbness, because the sciatic nerve is being irritated — usually by a bulging disc, a tight piriformis muscle, or a narrowed space in the spine. It is a symptom, not a diagnosis, so the first job is working out which of those it is.

Physiotherapy is first-line treatment for most sciatica. At home the physiotherapist assesses your back, hips and nerve tension, then uses a mix of hands-on release, nerve gliding, targeted mobility work and a short daily exercise set. Most people notice the leg pain easing within the first two or three sessions; the plan then shifts to strengthening so it does not return.

How many sessions does sciatica physiotherapy take?

Most courses are 4–8 sessions over 3–6 weeks. Acute flare-ups often settle faster; long-standing sciatica with weakness takes longer. We tell you our estimate after the first assessment.

Will the first session help the pain, or just assess?

Both. The first hour includes hands-on treatment and nerve-gliding work, so most people leave with less leg pain and a plan.

Do I need a scan or a doctor's referral first?

No referral is needed. A scan is only useful if there are red flags or the pain is not improving after a few sessions — the physiotherapist will say if that applies to you.
