About the Test
Total T4 measures thyroxine, the main hormone released by the thyroid gland, counting both the portion bound to proteins and the small free-circulating fraction. It gives a broad picture of thyroid output and is often paired with TSH to screen for an overactive or underactive thyroid. Because pregnancy, oral contraceptives and other conditions change protein levels, doctors sometimes prefer Free T4, but Total T4 remains a useful and widely used overview of gland activity.
