The free androgen index (FAI) is a calculated measure of how much active testosterone is available in the blood, worked out from total testosterone and the protein that binds it (SHBG). It is used mainly in women to assess symptoms of androgen excess such as excess hair growth, acne or irregular periods, and it is a helpful marker when investigating conditions like polycystic ovary syndrome. The index gives a clearer picture than total testosterone alone.
